Our clinical programming integrates proven therapeutic approaches, including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care methodologies.

Each modality is selected based on research demonstrating effectiveness for substance use disorders, mental health conditions, and co-occurring trauma, which we recognize is present in all our clients.

Every client receives individual therapy at least weekly with licensed therapists (LCSW-level), with additional sessions available as clinical needs dictate.

Rapid psychiatric assessment

Within 2 hours of admission, our psychiatrist conducts a comprehensive psychiatric evaluation. This accelerated timeline—far exceeding the industry standard of 72+ hours—ensures clients receive appropriate medication-assisted treatment and clinical interventions without delay.

This rapid response sets the foundation for successful recovery by addressing immediate psychiatric needs and stabilizing clients quickly.

Individualized treatment planning

Your recovery journey is uniquely yours. Our CEO personally creates treatment plans during the intake process, working alongside licensed clinical therapists who conduct same-day assessments.

Each client meets with a licensed therapist, psychiatrist, and case manager all on day one. We offer flexible treatment tracks—including substance use disorder primary, mental health primary, dual diagnosis, and mental health-only options—with the ability to transition between tracks as your needs evolve throughout treatment.

On-site medical detoxification

We provide comprehensive medical detoxification services at our dedicated detox location on the adjacent street in Carlsbad Village. Our 24/7 medically supervised detox program safely manages withdrawal from alcohol, opioids, stimulants, benzodiazepines, and prescription medications over an average of 5-10 days.

Comprehensive medication-assisted treatment

We embrace a medication-assisted treatment approach rather than demanding 100% abstinence from all substances. Our medical director works closely with each client to determine appropriate psychiatric medications and MAT options to support recovery.

We utilize GeneSight pharmacogenomic testing to optimize medication selection based on your genetic profile, ensuring the most effective treatment with minimal trial and error. For clients on our mental health track, medications like Ativan are available as clinically appropriate.

Holistic wellness integration

We recognize that healing encompasses mind, body, and spirit. Our programming integrates holistic therapies, including equine therapy, sound bowl sessions, yoga, and breathwork, alongside traditional clinical treatment.

We provide on-staff nutritionists and dietitians for clients with eating disorder histories, personal trainers for fitness programming, and an executive chef who prepares custom fresh juices daily.

Recovery affects entire family systems, but we recognize that family dynamics can be triggering during early treatment. We offer family therapy sessions on a case-by-case basis, prioritizing client stabilization before introducing family involvement.

We provide Wednesday night alumni meetings where families can connect with our recovery community, and many clients maintain relationships with our psychiatrist for ongoing prescriber support after discharge.

We maintain full kitchen facilities and provide custom nutrition rather than institutional food. This personalized approach extends to treatment duration—while our average PHP length is 4 months (exceeding 30-90 day programs), we adjust based on your insurance coverage, clinical progress, and individual needs, with some clients staying 2 weeks and others remaining 17 months.

We offer telehealth options for out-of-state clients and continuing care, work with multiple insurance providers, and provide supportive housing for 21 out of our average 23 clients.

We assess Adverse Childhood Experience (ACE) scores, which range from 7-10 in our population, and integrate trauma treatment simultaneously with substance use disorder and mental health treatment rather than addressing them sequentially.

How we got started

Filling the gap for truly individualized care

516 Recovery Center was founded by Samantha Paige, who brings both extensive recovery housing experience and a profoundly personal understanding of what makes treatment truly effective.

After managing sober living for one year and operating a standalone sober living facility for seven years, Samantha identified a critical gap in the addiction treatment landscape: the “heads and beds” mentality that treats clients as numbers rather than individuals with unique needs.

A New York native from Long Island with professional experience at Fox News, Samantha brought a demand for excellence and individualized attention to an industry that often operates on assembly-line principles.

She opened 516 Recovery Center on September 7, 2022, in Carlsbad Village by the beach, intentionally creating a treatment environment where the CEO personally answers intake calls, creates customized treatment plans, and maintains direct involvement in client care.

Samantha’s vision extended beyond creating just another treatment center. She built an organization where staff ratios enable genuine individualization (1:1 overall, 1:8 licensed clinicians), where clients see a psychiatrist within 2 hours rather than waiting days, where weekly request sheets allow personalized accommodations, and where pets are welcomed as part of the healing process.

Her commitment to the full continuum of care led to opening a dedicated medical detoxification facility on the adjacent street, eliminating the need for external referrals and ensuring seamless transitions throughout treatment.

Originally conceived as an all-women’s program, 516 Recovery Center has expanded to serve all genders while maintaining the personalized, boutique-level care that defines Samantha’s approach.

    We offer GeneSight pharmacogenomic testing to optimize medication selection, eliminating the trial-and-error approach of traditional psychiatry. With separate treatment tracks for substance use disorder primary, mental health primary, and mental health-only clients, we provide truly individualized psychiatric care rather than one-size-fits-all programming.

  • Medical detoxification services

    Our dedicated detox location provides 24/7 medically supervised withdrawal management for alcohol, opioids, stimulants, benzodiazepines, and prescription medications. With round-the-clock nursing and physician oversight, clients receive comfort medications for symptom management and medical stabilization over an average of 5-10 days.

    Unlike facilities requiring external detox referrals, our integrated approach ensures seamless transition from medical detoxification directly into PHP or IOP programming, maintaining continuity of care and therapeutic relationships throughout the treatment journey.

  • Trauma-focused therapeutic integration

    We assess Adverse Childhood Experience (ACE) scores and recognize that trauma—whether sexual, emotional, physical, or financial—is present in all our clients. Rather than requiring clients to achieve sobriety before addressing trauma, we integrate trauma treatment simultaneously with substance use disorder and mental health treatment.
